Inside the Fight to Control Malaria

In 2022, malaria killed an estimated 608,000 people worldwide, according to the World Health Organization โ€” and about 76% of them were children under five, mostly in sub-Saharan Africa. Yet malaria is entirely preventable and treatable. The gap between those two facts is exactly what the field of epidemiology exists to close: tracking how a disease spreads, testing what stops it, and turning that evidence into programs that save lives.

It's Not the Mosquito Bite โ€” It's the Parasite

Malaria isn't caused by mosquitoes themselves; it's caused by a single-celled parasite from the genus Plasmodium, which mosquitoes merely carry and inject when they bite. Only female Anopheles mosquitoes transmit malaria, because only females need blood meals to produce eggs. Once injected, the parasite travels to the liver, multiplies, then bursts into the bloodstream and infects red blood cells โ€” causing the fever, chills, and fatigue that define a malaria case, typically 10-15 days after the bite.

Four Tools Epidemiologists Track

Malaria control programs are built and measured around four core interventions. Insecticide-treated bed nets (ITNs) block mosquitoes from biting sleepers at night, when Anopheles mosquitoes feed most. Indoor residual spraying coats walls with insecticide that kills mosquitoes that land there. Rapid diagnostic tests (RDTs) let health workers confirm malaria in minutes using a finger-prick blood sample, instead of waiting days for a lab microscope. Artemisinin-based combination therapies (ACTs) are the standard treatment drugs, combining artemisinin (originally isolated from a Chinese herb, sweet wormwood, by chemist Tu Youyou, who won a 2015 Nobel Prize for the discovery) with a partner drug to prevent resistance.

Reading a Case-Rate Number

Epidemiologists often report malaria burden as 'cases per 1,000 population per year.' A country moving from 400 cases per 1,000 to 40 per 1,000 over a decade shows a 90% reduction in transmission โ€” the kind of trend that signals control programs are actually working, not just anecdotes of fewer sick neighbors.

Case Study: Sri Lanka's Road to Zero

In 1999, Sri Lanka recorded over 264,000 malaria cases. The country combined all four tools above with intensive case surveillance โ€” mobile teams tracked down every single reported case, tested the patient's family and neighbors, and sprayed the exact area where transmission occurred, rather than spraying entire regions. Cases dropped to zero locally transmitted cases by October 2012, and the World Health Organization officially certified Sri Lanka as malaria-free in September 2016 โ€” a model now studied by other countries pursuing elimination.
